therapy service form

Our therapy team consists of Occupational Therapists, Physiotherapists, a Moving and Handling Advisor, Dietitian and Therapy Assistants and is overseen by our Head of Therapies.

We provide therapy either in our Therapies Suite on the ground floor where we have access a purpose built clinical area with specialist equipment or up on the households, either in communal areas or in private rooms as well as the garden. Our aim is to work collaboratively to identify what is important to you so that we can devise a joint goal plan to support the areas you’d like to achieve. We use a variety of approaches including rehabilitative, with the aim of regaining function, compensatory with the aim of finding alternative ways of doing the things that are important to you or maintenance to maintain a level of function or independence. Our therapists wear blue tops so you can identify us easily.

what is included:

  • Assessment when you first arrive. This is a chance for us to get to know you, understand your needs, support with setting up your room so you can use it well and provide standard equipment where needed. If you need help with moving around (e.g. moving from bed to chair), we will complete a moving and handling assessment together so we know the best way we can support you
  • We can attend multidisciplinary review meetings with you and your family to discuss your therapy input and progress
  • We offer a review if your functional ability has changed e.g. on return from hospital or if you fall
  • Groups e.g. exercise or movement classes
  • Access to a Dietitian.

additional services:

You may wish to participate in a one-off session or a series of sessions working towards a particular goal on top of what we provide as standard. We can offer tailor made programmes in-house for an additional cost. If you’d like to consider this, please mention it to a member of the household team who will inform us. Tailored sessions can be arranged and charged accordingly.

we will:

  • Meet with you to determine what you’d like to focus on and your goals
  • Jointly agree a programme including the type of therapy and frequency of sessions
  • Build-in regular reviews so we can see if your goals are being achieved
  • We are happy to involve your family or friends if you’d like
  • Agree the cost and ask you or your power of attorney/deputy to consent and sign
  • Provide a summary of your therapy when programmes/sessions end.

 

please note the following:

  • You can stop sessions at any time however any started one to one session will be chargeable.
  • You may not see the same therapist every session however we are a close team with daily handover so you’ll soon get to know our team well
  • This does not prevent you from choosing to access statutory services instead if you wish
  • This does not prevent you from arranging your own private therapy. If you decide to do this, please talk to the household lead directly to discuss further. We do not offer mixed packages of in-house and private therapy other than the standard offer unless there is a very distinct and specialist need which is assessed on merit
  • Please direct any questions regarding insurance to your insurance company prior to committing to the sessions.

if you’ve never used therapy services before:

Examples below are some of the areas we can help you with though the list isn’t exhaustive and we’re always happy to discuss further.

  • Walking, muscle strength, joint range of movement/flexibility and balance
  • Falls prevention
  • Cognitive and perceptual rehabilitation including strategies for helping with memory
  • Adapting activities to make them easier or possible e.g. eating, washing or dressing
  • Postural care, specialist seating and 24 hour programmes
  • All activities of daily living including sleeping
  • Non-pharmacological symptom management e.g. managing breathlessness or fatigue
  • Anything you need or want to do that has meaning to you but find you can’t do anymore. Please let us know and we’ll be happy to try and help.
